Medical Release
The medical information regarding allergies, medications and restrictions, which appears as part of the online registration process is required.

Anyone mailing in a hardcopy registration must download and submit a medical release form.

Photo/Video Release
By participating in our Coerver Coaching program, gives permission for the staff of First Touch Futbol LLC, dba Coerver Coaching to use any photos taken of campers/students during camp/team training sessions to be used in advertising or promoting our camps or training sessions. This includes, but is not limited to, pictures used in brochures, seminars, expositions, or posted on Coerver related websites, including but not limited to, www.coerver.com or facebook. All photos and likenesses are property of First Touch Futbol LLC, dba Coerver Coaching.
